Inform OLM (admin@olmschool.org) of your interest in volunteering so we can initiate an invitation link to be forwarded to your email. This email will ask for your consent to run the background check. Completed every 5 years.
BCI Disclaimer Form
Complete form. Notary Public required. Attach a color copy of the FRONT and BACK of your driver’s license. Completed every 3 years or upon request.
OLM Volunteer Application
Complete form every year.
Certification of Completion-Safe Environment Training
This is to be signed after completion of the Safe Environment Training. Completed every 3 years.
